Your important rights

• Access and copy your records.
• Request corrections (amendments).
• Request limits on certain uses/disclosures.
• Request confidential communications (alternate phone or secure email).
• Receive an accounting of disclosures.
• Request a paper copy of the full Notice.
• Restrict disclosure to a health plan when you have paid in full. If you pay in full out of pocket for a service, you can require that information about that service not be disclosed to your health plan for payment or health care operations, and that request must be granted (45 CFR 164.522(a)(1)(vi)). This practice files no insurance claims, so it rarely arises. Where it does apply is a prior authorization, which is a disclosure to your plan.
• Be notified of a breach. If your unsecured health information is ever breached, you have the right to be told (45 CFR 164.520(b)(1)(v)(A)).

Telehealth & prescribing

Telehealth services are available only when you are physically located in North Carolina. Prescribing follows state and federal law; some medications may require an in-person visit or additional documentation.
