The Site provides general information about the practice, services, and resources. The Site is for informational purposes only and is not intended to provide medical advice, diagnosis, or treatment.

3. No Doctor-Patient Relationship

Use of this Site, including submission of forms or emails, does not create a doctor-patient relationship. A doctor-patient relationship is established only after an in-person or telehealth consultation has occurred and any required intake/consent forms have been completed.

4. Appointments, Forms & Patient Communications

Any appointment requests or information you submit via online forms may be used to contact you to schedule care. Do not include emergency or sensitive medical details in unsecured forms or emails. If you are experiencing an emergency, call 911 or go to the nearest emergency room.

5. Intellectual Property

All content on the Site — text, graphics, logos, images, digital downloads, and software — is the property of Dr. Shantanu Baghel or its licensors and is protected by copyright and other intellectual property laws. You may view and print pages for your personal, noncommercial use only. You may not copy, reproduce, republish, upload, post, transmit, or distribute Site content for commercial purposes without prior written consent.
